If you are a minor , please print out the following form ,complete it and bring it with you along with the consenting parent or guardian.As of July 2010 it is now Illegal to tattoo a Minor in the State of Minnesota. Piercings can still be done with Noterized,Writen consent and the parent being present( no Genital, Breast or Tongue piercings on anyone under the age of 18 yrs.
